Most plan comparison starts with the premium. That is the wrong end. The question that decides whether cover works when you need it is whether the hospital you would actually be taken to sits inside the network attached to the plan you bought.

This site works that question metro by metro, using the carriers and plan types filed in the CMS Plan Year 2026 landscape file. It does not publish lists of which insurance a hospital accepts, because those change mid-year and go stale without warning. What it does is show you what is knowable, explain what your plan type implies, and check the specific question for you.

In 13 of the 27 metros on this site — Austin, Cincinnati, Cleveland, Columbus, Dallas-Fort Worth, Houston, Indianapolis, Milwaukee, Omaha, Raleigh-Durham, Salt Lake City, San Antonio and Wichita — not one marketplace plan is a PPO. Every plan is an HMO, EPO or POS, which means out-of-network care is generally covered only in a true emergency. There is no fallback if you pick the wrong network.

In the other 14, a PPO is sold in every county and a wrong guess is merely expensive. Same premium on the listing page. Completely different exposure.
