Who files marketplace plans in Fort Wayne
Three insurers filed 2026 marketplace plans in Allen County in the CMS Plan Year 2026 file: Ambetter Health, Anthem Blue Cross and Blue Shield and CareSource (leaving after 2026). The lowest-cost Silver plan for a 40-year-old is about $506 a month before any premium tax credit. Plan types filed here: HMO and POS.
No marketplace plan sold in the Fort Wayne area is a PPO. HMO plans pay nothing outside their own network except in a genuine emergency, so if the hospital you want is not in the plan’s network there is no out-of-network benefit to fall back on. A POS plan may pay part of out-of-network care, at a higher cost — check its summary of benefits before you count on it.
For 2027: CareSource is leaving the marketplace here. If your plan is from CareSource, it ends December 31, and the plan you move to may use a different network. Choose by December 15 for cover that starts January 1.

How to check a hospital against a plan
- Start from the plan, not the insurer. Get the full plan name and its network name from the marketplace listing.
- Look the hospital up in that network in the insurer’s directory for the 2027 plan year, not the current one.
- Call the hospital’s billing office and read them the plan name. They can tell you whether they are contracted with that network.
- Check your doctors separately. A hospital being in a network does not put every doctor who works there in it.
- Write down the date and who told you. Contracts change during the year.
